I certify that the information provided on this roster is accurate to the best of my knowledge and that the players listed above are members of the team participating in the identified Prospect Network event.

I understand that each player must have a completed Prospect Network Participant Waiver and Media Release before being eligible to participate.

I understand that participants under 18 must have the required form completed and signed by their parent or legal guardian. Participants who are 18 years of age or older may complete and sign their own participant waiver.

I agree to notify Prospect Network of any roster additions or changes before the added player participates in an event.
